1. Declaración Preliminar

La Guardia Costera de EE.UU. llevó a cabo esta investigación bajo su autoridad legal para esclarecer el colapso del TITAN ocurrido el 18 de junio de 2023, que resultó en la muerte de cinco personas. La investigación se realizó en colaboración con organismos como el FBI, la NOAA, el NTSB y autoridades de Canadá, Reino Unido y Francia. Se realizaron audiencias públicas y se recopilaron testimonios de 26 testigos. El informe no puede ser usado como prueba en procedimientos civiles o administrativos, excepto por el gobierno de EE.UU.


2. Embarcación Implicada en el Incidente

El TITAN era un sumergible experimental de 22 pies de largo, propiedad de OceanGate Inc., construido en 2021, con un casco de fibra de carbono pegado a segmentos de titanio. No estaba registrado, certificado ni clasificado por ninguna entidad oficial. Las cinco personas fallecidas a bordo del TITAN fueron:

  • Stockton Rush (EE.UU.): CEO y piloto del TITAN.

  • Paul-Henri Nargeolet (Francia): experto en el Titanic.

  • Shahzada Dawood (Pakistán/Reino Unido): pasajero.

  • Suleman Dawood (Pakistán/Reino Unido): pasajero.

  • Hamish Harding (Reino Unido): pasajero.

Todos murieron instantáneamente por la implosión a gran profundidad.


4. Hallazgos de Hecho

El TITAN participaba en una expedición turística y científica al pecio del Titanic. Durante su 88.ª inmersión, el 18 de junio de 2023, perdió comunicación mientras descendía a unos 3.346 metros. Segundos después, sufrió una implosión catastrófica.

OceanGate operaba fuera de los marcos regulatorios tradicionales, sin certificaciones ni análisis de ingeniería suficientes. El casco de fibra de carbono no fue evaluado adecuadamente ni fue diseñado conforme a estándares aceptados. El sistema de monitoreo en tiempo real no fue capaz de predecir el fallo estructural. La empresa tampoco protegió adecuadamente el casco entre temporadas ni investigó eventos acústicos previos que podrían haber sido señales de alerta.


5. Análisis

Se identificaron múltiples fallos graves:

  • Falta de análisis estructurales y pruebas de resistencia del casco.

  • Uso de materiales no certificados (fibra de carbono, ventana acrílica).

  • Sistema de monitoreo mal implementado.

  • Desestimación de advertencias internas y externas sobre seguridad.

  • Cultura laboral tóxica, sin protocolos adecuados de seguridad.

  • Falta de supervisión externa y entrenamiento formal del piloto.

  • Presentación engañosa de los pasajeros como "especialistas de misión".

El análisis concluye que estos factores contribuyeron directamente al desastre.

.1. Diseño y materiales inadecuados

  • El TITAN usaba un casco de presión hecho de fibra de carbono enrollada, un material nunca antes aprobado para estructuras de presión de gran profundidad.

  • Este diseño no fue certificado ni verificado por ninguna autoridad independiente como DNV o ABS.

  • La unión del casco de fibra de carbono con extremos de titanio generó diferencias de expansión térmica y fatiga estructural, lo que contribuyó a su debilitamiento progresivo.

  • La ventana de visualización también fue hecha de un material acrílico no diseñado para las profundidades a las que se sumergía el TITAN.

  • No se realizaron pruebas exhaustivas de presión o resistencia cíclica (fatiga) que validaran la capacidad del casco para soportar repetidas inmersiones a grandes profundidades.

  • Los expertos de ingeniería dentro y fuera de la empresa habían advertido que el diseño no tenía un margen de seguridad estructural adecuado.

  • Se ignoraron incidentes acústicos (ruidos o "pings" estructurales) durante inmersiones anteriores que sugerían daños acumulativos en el casco.


5.3. Implementación fallida del monitoreo en tiempo real (RTM)

  • OceanGate promovía un sistema de monitoreo en tiempo real de la salud estructural del casco, como su innovación clave de seguridad.

  • Sin embargo, este sistema no tenía parámetros claros de seguridad, ni protocolos establecidos para abortar la misión si se detectaban anomalías.

5.4. Cultura organizacional y entorno laboral tóxico

  • El informe describe un ambiente de trabajo disfuncional donde se desalentaba el cuestionamiento de decisiones y se despedía al personal que expresaba preocupaciones de seguridad.

  • Se promovía una cultura de riesgo e innovación a cualquier costo, más alineada con el sector tecnológico que con la ingeniería marítima tradicional.

  • Ingenieros, empleados e incluso asesores externos expresaron sus reservas de manera formal e informal, pero sus advertencias fueron minimizadas o ignoradas.


5.5. Supervisión inadecuada del piloto y operaciones

  • El CEO de OceanGate, Stockton Rush, actuaba como piloto principal, sin haber pasado por entrenamiento formal o certificación de pilotaje submarino.

  • No existía un plan de emergencia claro ni simulacros regulares.

  • El control del sumergible se realizaba con un controlador de videojuegos modificado, símbolo de una actitud poco profesional hacia el diseño crítico.


5.6. Clasificación errónea de los pasajeros

  • OceanGate clasificaba a los pasajeros como "Especialistas de Misión", una designación ambigua que buscaba evadir requisitos regulatorios para el transporte comercial de personas.

  • A pesar de ello, los pasajeros pagaban cuantiosas sumas (hasta $250,000) por participar, lo que en la práctica representaba un servicio turístico comercial.


5.7. Falta de regulación específica

  • El TITAN operaba en aguas internacionales, aprovechando vacíos legales y la ausencia de jurisdicción directa de entidades como la Guardia Costera de EE.UU.

  • No existían leyes claras sobre cómo regular vehículos sumergibles experimentales de este tipo.


6. Conclusiones

El informe concluye que la tragedia del TITAN no fue un accidente inesperado, sino una consecuencia evitable de una cadena de decisiones irresponsables, falta de supervisión técnica, presión empresarial y una cultura de trabajo que priorizó la innovación y el marketing por encima de la seguridad y la ciencia de ingeniería sólida.

La causa principal fue el colapso estructural del casco de fibra de carbono, lo cual provocó la implosión instantánea. OceanGate ignoró principios de ingeniería fundamentales, omitió mantenimiento crítico, desestimó incidentes anteriores y operó con una cultura de seguridad deficiente. Se destacó la ausencia de regulación efectiva para sumergibles de diseño novedoso.


7. Acciones Tomadas Desde el Incidente

Desde el incidente, se han iniciado acciones para revisar y posiblemente reformar el marco regulador sobre sumergibles comerciales, especialmente aquellos de diseño no tradicional.
